British Woman Dies After Having Injections for Buttock Augmentation

I say and write it often- there are only 2 safe and effective ways to enhance your buttocks: buttock implants, and via the Brazilian Butt Lift technique. Anything else (no matter who is doing it for you, or how many friends you have who swear by it) is simply dangerous.

Of equal importance- if your goals are to be safe and happy with your outcome (duh!), be sure your surgery is performed by a surgeon certified by The American Board of Plastic Surgery. Think you can take this for granted? In Orlando alone, there are several ER doctors, a few gynecologists, and an eye doctor (!!) all marketing themselves to patients as liposuction experts.

New Micro-sites for Your Educational and Reading Pleasure!

I believe very strongly that educated patients are happier with their outcomes and more compliant with their instructions- and so work very hard to help my patients understand ALL of their options, the pros and the cons of each. 

Regular readers of this blog also know how extremely seriously I take my responsibilities to the community as a Board Certified Plastic Surgeon- I think it is very important in our current environment (what with ER doctors marketing themselves as "premier" providers of liposuction and eye doctors trying to fool you into thinking they are plastic surgeons...) to also help patients separate fact from fiction when it comes to aesthetic treatments. There is too much snake oil being sold in Central Florida...

So, having said all of that, I am very pleased to announce the launch of 2 micro-sites (small websites) to help educate our patients on some of their aesthetic options (with a few more to come!):

CNN reports: Curves and Lips are "In"

While it certainly is no secret to me, CNN is reporting this morning that the "ethnic" look of a full curvy body and full lips is now the most sought after aesthetic in the entertainment industry and suggests that the paradigms may be shifting....

As examples, they use Beyonce, Shakira, Christina Hendricks, Scarlett, and J. Lo-- all great examples to be sure, but I would add the great many young women we all encounter in everyday life to this list....

While I certainly still see lots of young women desiring liposuction or other forms of body sculpting, the majority of these patients are more interested in my ability to shift volume from one part of their body to another (the Brazilian Butt Lift is now among the most requested in my practice- to see why, see these photos:

I'm glad for the change myself- super-fitness is probably unrealistic for most women, and it is therefore unfair to bombard our sisters, wives, girlfriends, mothers, etc with images meant to demonstrate an ideal that is unattainable or unsafe for them to attempt.

The Six Women in New Jersey are Unfortunately Only the Tip of The Iceberg

Amazingly, the most common reaction to the fraudulent and dangerous activity that led to 6 women in New Jersey having diluted bathtub caulk injected into their buttocks for augmentation is some form of "those women must be dumb... that could never happen to me".

What's fascinating to me about this is that women seeking plastic surgery here in Orlando are duped daily, and with just as much on the line we just don't hear about it... Why?

The critical factors usually have to do with one or more of the following:

  • They never find out they were treated irresponsibly or by someone without proper credentials (that is, they are lucky to not be injured)
  • They suffer not a true injury, but a poor outcome due to the provider's lack of legitimate training- which makes it much more difficult to complain to authorities.
  • They are injured, but the injury is minor enough that a trip to the ER is not necessary, and so is not reported to authorities.
  • They are injured, but intimidated into submission by the "provider" in question.
The last one is very common here in Orlando. There is an eye doctor in town who completed a fellowship in plastic surgery of the eye and markets himself aggressively as a liposuction expert. I see many of his patients after they have been injured (psychologically, physically, or both) by him, and I must say I am struck by how often these (usually very intelligent) women are absolutely surprised to learn that he is NOT a Board Certified Plastic Surgeon.

I have asked why they assumed he was, and their response is always that he represented himself to them as a Board Certified Plastic Surgeon. 

If not legally fraud, no reasonable person could argue that this is not ethically and morally fraud. Misrepresenting your medical education and training ought to be against the law....

Unfortunately, until patients complain to the Florida Board of Medicine, his behavior will not be curbed.

Just as shocking (to me) however, is how often these intelligent, injured women are absolutely uninterested in filing a complaint. Why?

Most often they say it's because they are embarrassed that they made the poor decision in the first place. 

They don't want the world to know they were duped by an eye doctor into letting him do surgery outside his scope of training on them. Others have said that at a prior visit at which they expressed concern to him, they were berated and threatened with consequences if they complained. 

The point of all of this is that there is a lesson here for anyone and everyone considering plastic surgery of any kind. And that is that any of us could fall victim to an unscrupulous doctor with a clever marketing plan. 

You have simply GOT to do your homework before you let someone do surgery on you (and YES, liposuction of any kind IS surgery).

The best places to learn about your surgeon's education and credentials are:

    • Allows you to immediately find out if your doctor is a Board Certified Plastic Surgeon
  2. The Florida Department of Health Website

    • This site allows you to learn all about where your doctor was educated, where he did his training, what specialty his training was in, what kind of medical practice he is licensed for in Florida, and even all about his criminal history and history of complaints and lawsuits.
Here is what a quick search taught me about the Orlando eye doctor who claims to be a body contouring expert and who's patients I see so frequently:

* His residency training was in OPHTHALMOLOGY, and was followed by a fellowship in OCULOPLASTIC and RECONSTRUCTIVE SURGERY (plastic surgery of the eye)

* He is Board Certified BY THE AMERICAN BOARD OF OPHTHALMOLOGY

Bottom line- do your homework. Find a well-qualified plastic surgeon for you plastic surgery... Even if you don't come to me, there are many real plastic surgeons in Central Florida, any of whom could give you a pleasant, safe experience.

Remember- the goal is to look good when all is said and done, but also to have been safe, and to have a pleasant experience you can look back on positively.

6 Women Hospitalized After Botched Butt Enhancement Procedures by Shady "Practitioner"

From the New York Post:
NEWARK, N.J. —  A black-market butt-enhancement "practitioner" is injecting New Jersey backsides with household caulk and other hazardous materials, according to New Jersey health officials who have alerted New York authorities to the rear and present danger.
Six Newark-area women in the last two months have been hospitalized with infections caused by injections of "a variety of unknown materials," said New Jersey Health Department epidemiologist Dr. Tina Tan.
Officials said they believe the injected mixture includes silicone, petroleum jelly and hardware-grade caulk.
After botched efforts to plump their rear ends like naturally well-endowed celebs such as Kim Kardashian, the victim's derrieres resembled "moonscapes" filled with lumps and craters, said ahospital source.
"What we've been hearing from the hospitals is that these women are presented with deep tissue infections and skin infections. Abscesses form in some cases," said Tan.
According to hospital sources, the six women were all from the Dominican Republic. The injection "treatments" occurred in hotels around the Newark area, the hospital source said.

Revision of your Breast Implant Surgery can bring Positive Change

Because I completed a fellowship (an additional year of training after core residency training, in some sub-specialty) in advanced breast surgery techniques, I see between 10 and 20 pairs of breasts a day. In the course of this experience, I have learned a great deal from my patients...

I have learned that what is a pleasing and fulfilling surgical outcome to one woman may not be to another. Indeed, often in the same day I will meet a patient who wants her D cup breasts reduced to B's, and 60 minutes later a woman who wants to trade her B's for D's. 

It's not for me to judge what is best for any individual- provided their goals are safe and reasonable.

I have learned that (for the most part) men and women have much different views on what constitutes a "natural looking" breast (no surprise there).

I have learned that when most women feel beautiful they are more engaged, self-confident, and outwardly beautiful to their friends, colleagues and the casual observer. I also believe (and have been told by patients) they are more energetic and ultimately successful, although this would be impossible to measure.


I have also learned that when women experience a poor outcome, they are amazing in their capacity to accept deformity. The patient in this photo had been living with this outcome for years before finally doing the research it took her to find me and have it corrected to this:


And that's why I am so passionate about educating patients to make the very best decision they can in choosing who does surgery on you. Even of you don't come to me (I say), please, please PLEASE investigate your surgeon's education, training, credentials, and OUTCOMES thoroughly and with a critical mind... If there is anything worse than not being happy with yourself, it would have to be having surgery, spending money, going through a recovery, and STILL not being happy with yourself.

It is simply not reasonable to expect all plastic surgeons to be created equally- human beings are variable in every way, medical schools vary in quality and the experiences provided, as do residency training programs.

Because so many women unfortunately make poor decisions, based on all of the wrong criteria (like price) there are an amazing number of really funny looking breasts out there. And here's the thing- after having the surgery, there's an Emperor's New Clothes kind of phenomenon that often occurs, where the patient expresses concern over their outcome to their surgeon and he/she tells them that their concern is unfounded and the outcome is the best that could have been achieved.

To be fair, this is sometimes the case. But often it is not, and the only reason the breasts could not be more attractive in the opinion of the patient's surgeon is because he/she simply does not have the understanding necessary to create the positive change the patient wants. And so most of these women, not wanting to be complainers, or just because they don't know better, tolerate the problems with their breast contour- sometimes for many years, before finding someone like me and pursuing the change they wanted in the first place.

Revision of less than ideal breast implant outcomes is very common in my practice, and I am very proud of the appearance we are able to achieve for our patients (many of whom as I say, have had problematic experiences in plastic surgery before coming), but even more of the positive effect this then has on their sense of happiness and well-being.

I guess the take home message would be that you should minimize your risks of feeling dissatisfied with your surgery by making smart decisions in the first place, but if you do end up with breasts you are not happy with- know that improvement or correction is possible.

Understanding the Brazilian Butt Lift

 In our Orlando Plastic Surgery and Skin Care Center, we LOVE the Brazilian Butt Lift- it allows a more complete, permanent, circumferential transformation of a woman's body, so that the buttock area is not only (or not necessarily) larger, but more shapely and in better proportion to the rest of her. We have seen it transform many patient's sense of self and beauty in such a positive way...

I have found, however, that there are many misconceptions and myths surrounding the procedure- so here's my best attempt to provide a basic education on the technique, what you could expect, what would be expected of you after surgery, how to maximize your chances of achieving the beautiful buttocks you desire, how to minimize the risks to your safety, the alternatives, and the risks associated with Brazilian Butt Lifting.

In the Brazilian Butt Lift technique, fat is removed from areas where it is in excess and contributing to an unfavorable contour, treated to prepare it for replacement into the body (described below), and then carefully grafted into the area to be augmented (in this case the buttock area). This is the key to understanding the enormous transformational power of the procedure- unlike other plastic surgery procedures, this technique really allows the realization of more significant change by altering not just the dimensions and appearance of the buttocks directly, but by also allowing for dramatic change in the relationship of the buttocks to the areas of the body around it, such as the waist, hip, and thighs.  The CONTEXT in which the buttocks is viewed changes- which, combined with the changes in the buttock area itself, is very powerful.
Removal of the fat is done by liposuction, and there are several important considerations here.

First of all, it is very important to realize that liposuction IS a surgical procedure- the benefits of liposuction cannot be denied, and under the proper circumstances, and in the appropriate hands, it is among the safest operations performed by Board Certified Plastic Surgeons.... but in the wrong hands, it can, and has been, fatal.

Within the past few months, here in my home state of Florida, a young woman died during liposuction being performed by a rehabilitation medicine doctor.

So while the best methods for choosing a safe and well trained surgeon are beyond the scope of this article (you can read more about that topic here: http://www.realself.com/article/Choosing-a-Plastic-Surgeon-Part-2), please understand that it is very important to your safety and chances of achieving your goal that you choose your provider well.

Secondly, the technique used to harvest the fat needs to be different than you would use if the only goal were to remove the fat (but not replace it into the buttock)- the goal here is not simply to remove the fat, but to also have it SURVIVE, in order to give you the permanently improved shape you desire.

This highlights the importance of finding a surgeon who is not only Board Certified by The American Board of Plastic Surgery, but also very experienced in this technique. The fat harvested is precious- you will never be able to get it back... if poor technique is used, resulting in poor success of the procedure, you will not have a second opportunity. Even with the best technique, approximately 20-25% of the fat will be reabsorbed over the first month after surgery, so most surgeons over-graft a bit, meaning that your buttocks may initially seem a bit too large- don't panic.

Some surgeons use drains, some do not- this is a personal preference.

The fat used may come from anywhere it is in excess, including the arms, abdomen, waist, hips, thighs, etc..

Once the fat has been treated and carefully grafted into the buttock area, the operation is completed and you are placed in a support garment for transport to the recovery area.

The average Brazilian Butt Lift takes me between 3 and 4 hours, as good technique is very labor intensive and tedious. Some surgeons may take longer, and some less time- again, it's all about finding a well-trained, Board Certified Plastic Surgeon who can keep you safe and give you the outcome you want.

The volume of fat transferred can vary widely depending on your goals, your examination, and the qualities of your fatty tissue noted at surgery. We have achieved our patient's goals with as little as 300 cc, and with as much as 1000 cc (per buttock cheek).

Achieving success is most appropriately measured individually.

With regard to recovery, the procedure is typically not very painful after the first day, with most of my patients describing a muscle soreness type of pain which is easy to control with medication. We would not want you participating in exercise or heavy labor for several weeks after surgery (3-4), but you may return to your everyday home life and personal care immediately.
The most important postoperative concern is that you NOT SIT ON YOUR BEHIND for at least a month. This is very important to avoid losing the fat or altering the shape of the buttocks achieved by your surgeon in the operation. Your surgeon will teach you ways of sitting that allow avoidance of this pressure (in other words, you can still sit- just not on your butt).
You will look better and notice the improvement immediately, but of course the improvement should be expected to evolve over time. Once the first month to 6 weeks have passed, your buttock volume and shape should have stabilized and your final result evident in this area- but the areas treated by liposuction will continue to improve and evolve for an additional 2 months on average, so your final result in all areas will take between 3 and 4 months.

For most patients with administrative positions, return to work is possible after 1-2 weeks. More significant physical job requirements may necessitate another week off before returning.

The risks of Brazilian Butt Lift surgery include those of any operation, those particular to liposuction, and the risks of fat grafting in volume.

General risks of surgery would include bleeding, infection, unfavorable scarring, changes or loss of sensation, asymmetry (where the two sides of your body don't look the same), hematoma or seroma (fluid collecting under the skin), the risks of anesthesia, and the risk that you don't get exactly what you wanted- in other words, just as with any other aesthetic procedure, no guarantee of the outcome can be responsibly given.

Risks of liposuction include the risk of injury to nerves, blood vessels, and organs, as well as contour irregularities that may or may not need additional treatment.

The risks of fat grafting include the risk that more of the fat than anticipated is absorbed or dies (fat necrosis), becomes infected, or causes other problems. The risk of fat embolism (where some of the fat gets into your blood vessels and travels to your lungs/heart/brain/other organs causing problems) should be understood, but should be extremely rare with good technique (I am not aware of this ever actually happening with a Brazilian Butt Lift).

All of these risks can be minimized and managed quite effectively by choosing a surgeon, facility, and anesthesiologist properly, as outlined above and in the accompanying article, "Choosing a Plastic Surgeon, Part 2".

With a healthy understanding of your goals, the procedure, its recovery, its risks, and most importantly how to choose the right situation for you, the Brazilian Butt Lift technique can safely and permanently improve the way you feel about your body.

Buttock implants are the only safe alternative to Brazilian Butt Lift surgery. They are not ideal, as they are not permanent and can be prone to infection, as well as the fact that they may not ever look or feel as natural as a buttocks created using your own fat- but for women who don't have enough fat for BBL and who don't want to (or cannot) gain weight, they are a reasonable option.

There are many unscrupulous providers out there making promises that cannot be kept safely- one of injuries I see most often in Central Florida results from the injection of liquid silicone or "hydrogel" into the buttock tissues for "safe, permanent" buttock augmentation "without surgery". There have been numerous cases of organ failure and death from this practice, and even those patients who do not experience these devastating problems still have a lifelong problem with pain and disfigurement- as once injected it is almost always impossible to remove the liquid polymer.

The Brazilian Butt Lift for Beautiful Buttocks








These are representative photos of a recent Brazilian Butt Lift patient. What a wonderful transformation. Making patients like this one so much happier with their bodies is what makes my profession so fulfilling for me, and it was a pleasure to know and help her.
